General information, not medical advice or an insurance coverage decision. MedraxDME operates as an out of network supplier. Coverage, supplier enrollment requirements and patient responsibility depend on the plan and an individual review.
Use the code as a starting point
HCPCS Level II uses alphanumeric identifiers for many medical items and services. A code can describe a reusable device, an orthosis, an individual supply or a bundled supply allowance. It is not a diagnosis and usually is not a unique manufacturer model number. The pages below explain those differences using the CMS: October 2026 HCPCS release and quarterly updates.
These educational summaries are paraphrases, not complete coding instructions. The clinician determines the appropriate equipment. The supplier and insurer must verify the actual product, applicable policy, documentation and benefit route. A code appearing here does not establish that MedraxDME can provide it under your plan.

- A4239: the CGM supply allowance, distinct from a receiver.
- E2103: a qualifying CGM monitor or receiver.

- L0650: the related spinal brace category supplied off the shelf.
- L1833: a rigid knee brace with adjustable joints.
- K0001: a standard manual wheelchair.
- E0601: a CPAP device.
- E0603: an electric breast pump.

Check the date and the decision
CMS updates its code files quarterly. Coverage policies and authorization rules have their own effective dates. For example, catheter definitions changed in 2026, while the L1833 authorization requirement has an October 28, 2026 start date. An old invoice or saved search result can therefore be an incomplete reference.
For a specific request, confirm the full product name, the code under review, the current policy and the required records. The CMS: Search the Medicare Coverage Database links to coverage documents. Ask the insurer to explain any patient responsibility before supply, and avoid changing an order yourself to fit a code.
MedraxDME supports insurance requests through a callback. Do not submit medical records or member numbers in the public form. Read the written order guide for the distinction between an order and the clinical evidence supporting it.
Questions and answers
Does finding a code mean my insurance covers the item?
No. Code identity, clinical need, benefit rules, supplier access and required documentation are separate checks.
