General information, not medical advice or an insurance coverage decision. MedraxDME operates as an out of network supplier. Coverage, supplier enrollment requirements and patient responsibility depend on the plan and an individual review.
A supply allowance, not a receiver
In the CMS: October 2026 HCPCS release and quarterly updates, A4239 covers the supplies and accessories for the qualifying CGM category, with one month corresponding to one unit of service. The associated reusable monitor or receiver is described separately by E2103. A brand name and a billing code answer different questions: one identifies the system, while the other describes the billing category.
Nonadjunctive refers to the device's authorized use in treatment decisions without routine confirmatory fingersticks. It does not remove instructions to use a blood glucose meter in specified situations. Follow the actual device guide and your clinician's monitoring plan.
Why the number of sensors can differ
The CMS policy treats the allowance as a bundle, including necessary sensors and transmitters. It is not permission to bill those included supplies again as separate items. See CMS: Glucose Monitors policy article A52464. A monthly allowance also does not mean that every system has an identical number of sensors, identical wear duration or the same package contents.
Ask the dispensing team to explain the prescribed model, period of supply, included components and expected refill process. If a sensor fails early, clarify whether the manufacturer replacement route applies before confusing that issue with a routine refill. Keep the exact model name available for the conversation.
Coverage still requires an individual review
The Medicare glucose monitor policy has clinical, practitioner visit and continued use requirements. A4239 does not bypass them or establish that a phone only configuration qualifies under the DME benefit. CMS: Glucose Monitors coverage policy L33822 and CMS: Glucose Monitors policy article A52464.
Before a supply arrangement is confirmed, ask which organization can serve the plan and whether all required records have been received. Do not calculate your own entitlement from a code on a website. Browse diabetes supplies to identify the product, then read the detailed CGM coding guide for the relationship between equipment and supplies.
Questions and answers
Does A4239 identify Dexcom or Libre?
No. It describes a category of CGM supplies. The prescribed system and the applicable product classification must be confirmed separately.
