General information, not medical advice or an insurance coverage decision. MedraxDME operates as an out of network supplier. Coverage, supplier enrollment requirements and patient responsibility depend on the plan and an individual review.

The professional team reviews requests Monday through Friday, 8 AM to noon and 1 PM to 5 PM Arizona time (MST). Confirm the intended recipient and the referral arrangement by phone before faxing records. Follow your organization’s privacy and disclosure procedures when transmitting patient information.
Use info@medraxdme.com for general coordination only. Ordinary email and the website callback form are not for medical records, member numbers, prescriptions or patient documents. The public form collects contact details to start a conversation.
Confirm the benefit and dispensing arrangement first
MedraxDME provides equipment through insurance billing and is an out of network supplier. A product appearing in the catalog does not establish payer approval, program enrollment, network participation or stock for a particular request. Confirm whether the patient’s plan permits the proposed supplier and whether a pharmacy or equipment benefit applies before directing a prescription.
For Medicare or Medicaid, verify the relevant enrollment and dispensing requirements separately. An NPI is an identifier, not proof of Medicare enrollment, accreditation or a payer contract. For managed care, ask the current plan about its supplier network and authorization pathway. If the arrangement cannot support the request, help the patient identify an eligible dispenser rather than assuming that an out of network review is available.
Patients and caregivers can request a callback. Clinical records should follow only after the responsible offices have confirmed the appropriate pathway.

The treating professional determines clinical appropriateness. The payer and eligible dispensing provider confirm the current documentation and processing requirements for the actual item.
Use current written order requirements
For Medicare DMEPOS requests, CMS describes a standard written order that identifies the beneficiary, item, applicable quantity, treating practitioner, order date and practitioner signature. Some selected items also require a face to face encounter, an order before delivery or prior authorization. Check the current lists for the exact item and date of service. Read the official CMS order guidance.
A downloadable checklist does not replace the signed order or supporting clinical record. Use your practice’s appropriate order workflow and the receiving provider’s confirmed requirements. Do not backdate an order, add a diagnosis that has not been established, or treat an unsigned catalog selection as a clinical prescription.
The order and records should describe the same equipment configuration. Clarify laterality, sizing, required accessories and the purpose of a replacement with the treating team when relevant. Verify which office will respond if additional information is requested.
Do not assume an old CMN form is required
CMS discontinued Medicare Certificates of Medical Necessity and DME Information Forms for claims with dates of service on or after January 1, 2023. Medical necessity documentation and other applicable requirements still remain. See Chapter 5, section 5.5 of the Medicare Program Integrity Manual.
A Medicaid program or another payer may use its own documentation or certification process. Ask which current form, if any, applies to that program and product. Do not use an old Medicare CMN merely because it appears in a search result or another supplier’s packet. The goal is an accurate, current request with relevant records, not the largest possible collection of forms.
Coordinate the referral through its next steps
- Clarify the request. Identify the product category, exact item and whether the request is new, a refill, a replacement or a change of therapy.
- Confirm the responsible provider. Establish the benefit route, participating dispenser and any authorization responsibility.
- Agree on record transfer. Verify the recipient before sending only the appropriate records through the confirmed channel.
- Track the handoff. Keep a professional contact, submission date and next action in the practice’s records. A fax transmission receipt is not confirmation of acceptance or coverage.
- Confirm the outcome. Resolve missing information and verify benefits, equipment availability, training and delivery arrangements before the care plan relies on a shipment.
For discharge planning, communicate the proposed discharge date to the responsible team and confirm the actual arrangements. This page does not promise a response time, authorization outcome or delivery date. Urgent clinical needs should be addressed through the treating team’s existing care process.
Business identity and verification
MedraxDME operates as MEDRAX LLC. Its public NPI is 1790528081, which can be checked in the official NPPES registry. An NPI identifies the organization; it does not establish program enrollment, clinical credentials, accreditation or acceptance by a particular insurance plan.
For a referral, verify the business identity and recipient along with the plan’s requirements. Use the professional phone number above for questions about where the request should go. Please do not send a full patient record to the ordinary email inbox to begin that conversation.
Questions and answers
Where can a clinical office send an agreed referral?
After confirming the recipient and referral arrangement, use the secure referral fax at (480) 372-3067. Call (480) 550-8566 for professional coordination. Follow your organization’s privacy procedures.
Can patients upload prescriptions or insurance cards on this website?
No. The public form collects contact details and optional equipment selections. Do not enter member numbers or medical information or use ordinary email for health records.
Are the PDF checklists prescriptions or signed orders?
No. They help an office prepare questions and relevant information. The treating professional must use the appropriate prescription and documentation workflow.
Does an NPI confirm that MedraxDME can bill my patient’s plan?
No. Verify program enrollment, plan participation and the proposed dispensing arrangement separately for every referral.
Should every Medicare DME request include an old CMN?
No. CMS discontinued those Medicare forms for dates of service from January 1, 2023. Current orders, medical necessity records and item specific requirements still matter; other payers may have different forms.
