General information, not medical advice or an insurance coverage decision. MedraxDME operates as an out of network supplier. Coverage, supplier enrollment requirements and patient responsibility depend on the plan and an individual review.
Identify the durable component
The CMS: October 2026 HCPCS release and quarterly updates lists E2103 for the qualifying CGM monitor or receiver. A4239 describes the related supply allowance. Neither number is a complete product name, and the receiver must be compatible with the system prescribed for the individual patient.
A receiver is not interchangeable with any screen that can display health information. Ask the clinician and supplier to identify the exact model, confirm that it works with the prescribed sensors, and explain how to use it. Do not order a second display solely because a code appears on an old claim.
Why a smartphone does not settle the DME question
CMS distinguishes a qualifying durable receiver or pump display from a system used only through a smartphone. A phone can be used alongside qualifying durable equipment, but phone only use does not meet the DME requirement described in the policy. CMS: Glucose Monitors policy article A52464.
This distinction is about the benefit arrangement. It is not a judgment that one display is clinically best for everyone. Discuss readability, alerts, charging, training and caregiver support with the care team. Confirm your actual use rather than assuming that owning an unused receiver automatically satisfies a coverage rule.
A replacement is a separate request
If your receiver is lost, damaged or incompatible with a proposed system change, tell the dispensing organization exactly what happened. A new sensor prescription does not automatically create approval for another receiver. Ask whether the issue belongs with manufacturer support, a replacement request or a plan review.
The CMS: Glucose Monitors coverage policy L33822 and applicable documentation rules still govern coverage. Ask who will obtain the written order and any supporting records before an arrangement is made. MedraxDME's diabetes catalog provides product information, while the full CGM code article explains the equipment and supply distinction in more detail. This glossary does not determine replacement eligibility.
Questions and answers
Can E2103 be used to identify any CGM app?
No. It is a monitor or receiver code within a defined equipment category. An app or smartphone is not automatically qualifying DME.
