General information, not medical advice or an insurance coverage decision. MedraxDME operates as an out of network supplier. Coverage, supplier enrollment requirements and patient responsibility depend on the plan and an individual review.
The brace and the fitting both matter
The CMS: October 2026 HCPCS release and quarterly updates describes a lumbar sacral brace providing control in two planes, with rigid front, back and side support. The rear support extends toward the T9 vertebral level. L0637 also specifies that the prefabricated item is modified to fit an individual by a person with appropriate expertise.
This is not simply another name for every back belt. A photograph, adjustable strap or familiar manufacturer name cannot establish the correct classification. The clinician should select the support needed, and the supplier must verify the device and fitting actually provided.
How L0637 differs from L0650
L0650 describes a related prefabricated brace category supplied off the shelf. CMS distinguishes minimal adjustment from fitting that requires additional expertise. See CMS: Spinal Orthoses policy article A52500. The difference is not a consumer quality ranking or permission to choose whichever code seems more favorable.
Ask who will fit the brace, what adjustments are expected, how comfort and fit will be checked, and whom to call if it does not fit correctly. Do not bend or alter rigid components yourself to reproduce a fitting service. Follow the clinician's wear instructions and the manufacturer's product guidance.
Documentation and authorization
The CMS: Spinal Orthoses coverage policy L33790 explains the clinical reasons that can support a spinal orthosis. CMS also lists L0637 among orthoses subject to required prior authorization. The applicable order, clinical records and authorization review must be completed as required for the actual supply. CMS: Current DMEPOS prior authorization requirements.
An authorization discussion does not establish that every patient with back discomfort qualifies. Ask the supplier for a specific list of missing records rather than rewriting an order yourself. Browse braces and supports and read how written orders work before an insurance request. A code explanation is not a prescription or individualized billing advice.
Questions and answers
Does adjusting a strap make a brace L0637?
Not by itself. The device features and the fitting actually performed must meet the applicable classification and documentation requirements.
