DME AND INSURANCE INFORMATION

What does HCPCS B4035 mean?

B4035 identifies the daily supply allowance associated with pump administered enteral nutrition. It is distinct from the pump itself, feeding tube and nutrients.

General information, not medical advice or an insurance coverage decision. MedraxDME operates as an out of network supplier. Coverage, supplier enrollment requirements and patient responsibility depend on the plan and an individual review.

A daily allowance rather than one retail kit

The CMS: October 2026 HCPCS release and quarterly updates describes B4035 as supplies for pump fed enteral nutrition, per day. CMS explains that this is a supply allowance rather than one universally defined package. The items used can differ according to the individual feeding arrangement. CMS: Enteral Nutrition policy article A58833.

Ask the dispensing team what supplies are included for the prescribed method and how they will be provided. Do not assume a product marked “feeding supplies” contains a pump, formula or every item needed for your care. Confirm the actual contents and compatibility with the clinical team.

The administration method matters

CMS distinguishes pump, syringe, gravity and other feeding supply categories. The supply allowance must match the method used, and the record must support a pump when one is ordered. CMS: Enteral Nutrition coverage policy L38955. B4035 should not be treated as a general code for all nutrition products or oral supplements.

The clinician determines the feeding method, formula and care plan. A code explanation does not provide instructions for feeding rates, tube care, flushing or changing equipment. Ask the treating team for training specific to the prescribed system and who to contact when a clinical or equipment problem arises.

Confirm equipment and supplies as a complete plan

The pump, tube, nutrients and supply allowance are different parts of the arrangement. Clarify which organization handles each part and which items are included in the allowance. This helps avoid gaps or duplicate requests when more than one office is involved. It does not authorize separate billing of supplies already included in a bundle.

MedraxDME's enteral catalog lists specific products. A syringe, tube or pole listing does not establish that the site provides a complete pump feeding program. Ask the team what can actually be arranged through your plan before changing an existing supplier.

Read the written order guide to understand how prescriptions and supporting records differ. Keep feeding records and insurance details out of the public callback form.
