DME AND INSURANCE INFORMATION

Understand Medicare CGM coverage before requesting supplies

A Medicare coverage rule and a supplier’s eligibility are separate questions. Confirm both before transferring a prescription.

General information, not medical advice or an insurance coverage decision. MedraxDME operates as an out of network supplier. Coverage, supplier enrollment requirements and patient responsibility depend on the plan and an individual review.

Begin with the official coverage rules

Medicare’s CGM coverage page describes possible coverage for people with diabetes who use insulin or have a history of problems with low blood sugar, along with training and practitioner evaluation requirements. The treating practitioner must evaluate and document the applicable criteria.

A diagnosis by itself does not answer every coverage question. The exact system, required records, ongoing follow-up and dispensing arrangement matter. Discuss the current requirements with your clinician and insurer rather than relying on a past approval or a friend’s experience.

Confirm supplier enrollment and the plan route

Original Medicare requires an enrolled supplier for covered DME. An NPI is an identifier, not proof of Medicare enrollment. Ask whether the supplier is currently eligible for the requested equipment and how assignment is handled. Medicare Advantage plans may use their own network and authorization processes.

MedraxDME has a public NPI record, but this page does not represent confirmation of Medicare enrollment or participation with a Medicare Advantage plan. Verify the actual supplier arrangement before sending an order. Our team can discuss an equipment request without promising a payable Medicare claim.

Separate sensor, receiver, refill and replacement requests

The prescribed sensor system and its receiver must be identified accurately. Ask whether the order is for an initial system, a routine supply refill, a receiver or a product issue. These requests are not interchangeable and may require different handling.

If you already receive CGM supplies, check the last covered supply period before changing suppliers. If a sensor fails early, contact the manufacturer through its official process and follow your clinician’s backup monitoring plan. An early failure does not automatically authorize a new insurance refill.

Prepare questions for your care team

  • Which current coverage pathway applies to my request?
  • Does my practitioner have the required documentation?
  • Which sensor and receiver or compatible display are prescribed?
  • Is the dispensing supplier eligible under my Medicare arrangement?
  • Are any authorizations or follow-up visits required?
  • What patient responsibility applies to the confirmed arrangement?

Do not send Medicare identifiers, glucose readings or medical records in the online form. Use the callback to establish the appropriate next step and secure documentation route.

Questions and answers

Does an active NPI prove Medicare participation?

No. An NPI identifies a provider. Medicare enrollment and the applicable supplier arrangement must be confirmed separately.

Does type 2 diabetes automatically rule out CGM coverage?

No. Eligibility depends on the current coverage criteria, clinical documentation and other applicable requirements, not the diabetes label alone.
