General information, not medical advice or an insurance coverage decision. MedraxDME operates as an out of network supplier. Coverage, supplier enrollment requirements and patient responsibility depend on the plan and an individual review.
Choose the prescribed G7 model
Our catalog includes Dexcom G7, Dexcom G7 15 Day and a G7 receiver kit. They are distinct items. Do not select a sensor only because its name resembles your current device. Ask your prescriber which model is intended, and confirm the matching receiver, smartphone and any connected insulin delivery system against the current manufacturer instructions.
If your prescription says G7 and you want G7 15 Day, have the prescribing office confirm the change before arranging supplies. A longer wear option is not an automatic substitution. The Dexcom user guides identify the intended users and supported operation for each system.
Check the benefit and supply channel
CGM supplies can follow a medical equipment or pharmacy benefit, depending on the plan. Dexcom describes both pharmacy and distributor access. Your insurer determines which applies to you and which supplier may dispense the product.
Ask whether the exact model is covered, whether an authorization is required, whether the receiver needs a separate order and when your current supply can be refilled. If you already receive sensors elsewhere, check the last billed period before transferring. MedraxDME’s NPI and state permit do not independently establish participation with your insurer.
Start with a insurance review
- Request a callback. Give your name, phone number and email. You can also choose a preferred contact time. Do not enter your diagnosis, insurance number or prescription in this form.
- Confirm the appropriate supplier route. Ask the team and your insurer whether the requested equipment can be supplied under your plan. A pharmacy benefit, contracted supplier or other dispensing route may be required.
- Arrange the documentation securely. Once the recipient and next steps are confirmed, your clinician can use our referral fax at (480) 372-3067. Do not send records to ordinary email.
- Review the result before proceeding. Confirm the exact equipment, any authorization, patient responsibility and fulfillment arrangements. A callback or prescription alone is not an approved order.
MedraxDME provides equipment through insurance billing and operates as an out of network supplier. An equipment listing does not establish plan participation. We do not guarantee approval or a particular patient cost.
Keep routine refills separate from device support
A sensor that fails early, an adhesion problem and a scheduled refill are different requests. Record the model and contact the manufacturer through its official support channel for a product issue. Follow your clinician’s backup monitoring instructions if readings are unavailable. Do not delay urgent medical care while waiting for an equipment callback.
For an insurance review, the useful starting information is the equipment you want to discuss and a reliable callback method. Our public form is deliberately limited to contact details; the team will explain the next appropriate step.

Questions and answers
Does submitting the form confirm coverage?
No. It requests a conversation. Your plan, prescription, supplier eligibility and documentation still need individual review.
Can I switch from G7 to G7 15 Day through this form?
The form cannot change a prescription. Your clinician must confirm the appropriate model, and the plan and supplier route must be checked.
