General information, not medical advice or an insurance coverage decision. MedraxDME operates as an out of network supplier. Coverage, supplier enrollment requirements and patient responsibility depend on the plan and an individual review.
Medical equipment and pharmacy routes differ
Insulin delivery systems are not all billed the same way. A durable pump, disposable Pod system and their ongoing supplies may use different benefit structures. The label “insulin pump” alone does not identify the correct claim pathway.
Medicare’s DME information explains the importance of eligible equipment and enrolled suppliers. Insulet describes Omnipod’s Medicare Part D route. These are separate processes. Ask the insurer about the exact prescribed system instead of assuming one rule applies across all pumps.
Ask the prescribing team to define the system
Keep the model, compatible supplies and any CGM integration clear. Ask who will arrange clinical education, how ongoing supplies will be prescribed, and who provides urgent troubleshooting. A product comparison or insurance callback is not pump training and does not authorize a treatment change.
If you want to change systems, ask whether your current pump is owned or rented and whether its service or replacement period affects the request. Your plan can explain any prior authorization or replacement rules. Do not assume that a newer model is automatically covered.
Review the complete ongoing arrangement
- Which benefit handles the pump and which handles disposable supplies?
- Must the order go to a contracted DME supplier or a pharmacy?
- Are a new prescription, clinical records or authorization needed?
- What supplies are compatible with the prescribed model?
- How are refills and manufacturer warranty issues handled?
- What patient responsibility should be confirmed before fulfillment?
Ask for the answer relevant to your actual plan, not another person’s experience. Existing authorization for one product does not establish approval for a different system.
Start with a insurance review
- Request a callback. Give your name, phone number and email. You can also choose a preferred contact time. Do not enter your diagnosis, insurance number or prescription in this form.
- Confirm the appropriate supplier route. Ask the team and your insurer whether the requested equipment can be supplied under your plan. A pharmacy benefit, contracted supplier or other dispensing route may be required.
- Arrange the documentation securely. Once the recipient and next steps are confirmed, your clinician can use our referral fax at (480) 372-3067. Do not send records to ordinary email.
- Review the result before proceeding. Confirm the exact equipment, any authorization, patient responsibility and fulfillment arrangements. A callback or prescription alone is not an approved order.
MedraxDME provides equipment through insurance billing and operates as an out of network supplier. An equipment listing does not establish plan participation. We do not guarantee approval or a particular patient cost.
Questions and answers
Does MedraxDME supply every insulin pump?
No such claim is made. Review the actual catalog and confirm the exact product, plan and dispensing route with the team.
Can I change my treatment through this website?
No. A clinician must direct treatment decisions and training. The public form is for a equipment conversation.
