DME AND INSURANCE INFORMATION

Discuss a move to MedraxDME without interrupting supplies

A supplier change should begin with eligibility and continuity checks. Keep your existing arrangement until the next steps are confirmed.

General information, not medical advice or an insurance coverage decision. MedraxDME operates as an out of network supplier. Coverage, supplier enrollment requirements and patient responsibility depend on the plan and an individual review.

Confirm whether a transfer is appropriate

Start by asking your insurer whether MedraxDME can supply the specific equipment under your plan. We operate as an out of network supplier. A plan may require a pharmacy, a contracted equipment supplier or a separately authorized arrangement. Do not cancel your current service solely because you have submitted a callback request here.

Check the exact equipment and accessories you currently use. Similar names do not establish interchangeable models, and a supplier transfer is not a clinical prescription change. Your treating professional should direct any change in equipment or use.

Check pending shipments and the last supply period

Ask your current supplier whether an order has already been processed or shipped. Confirm the period covered by the last supply and any refill timing rules. Two simultaneous requests can create confusion, duplicate shipments or claims that need correction.

If equipment is rented, ask who owns it and how a transfer affects the rental arrangement, service and maintenance. Do not return or discontinue essential equipment until a safe, confirmed plan is in place with the relevant provider and care team.

Coordinate the prescription and records

Your clinician may need to send a current order and supporting records through an appropriate channel. After confirming the recipient, clinics can use MedraxDME’s referral fax, (480) 372-3067. The website form is for contact details and optional equipment selections and is not a prescription transfer service.

Ask who is responsible for any new authorization, whether an existing authorization can be used, and how you will receive the review result. No transfer is approved merely by adding equipment to a saved website list.

Agree on the next step before changing service

  • Exact model and components confirmed with your clinician.
  • Plan and supplier eligibility confirmed.
  • Pending shipments and supply dates reconciled.
  • Required records and authorization reviewed.
  • Patient responsibility and fulfillment arrangements explained.
  • Support contacts and continuity plan understood.

Our team can start that conversation. A callback is not a guarantee of approval, supply or delivery. Keep urgent medical and equipment safety concerns with your treating team and the manufacturer’s appropriate support service.

Questions and answers

Should I cancel my existing supplier before submitting a request?

No. Confirm eligibility, records, pending supplies and continuity arrangements before changing an existing service.

Does a saved equipment list transfer my prescription?

No. It is a list on your device. The prescriber and relevant suppliers must coordinate any actual order or transfer.
