Understand the January 2026 update
Pennsylvania’s CGM bulletin effective January 5, 2026 requires prior authorization for CGM prescriptions. Its medical necessity review includes a diabetes diagnosis or recent antidiabetic medication use. For a nonpreferred product, the update allows consideration of compatibility with an insulin delivery device, alongside the existing preferred product failure pathway.
This is useful when a specific sensor is part of an integrated system. Ask the prescribing office to explain the compatibility issue accurately in the request. Do not substitute products on the strength of similar names or claim that a device failed when it has not. The reviewer needs the actual clinical and product circumstances.
Confirm where the request is being processed
The bulletin applies its guidelines to both fee for service and managed care. It directs managed care questions to the member’s organization. Before sending records, ask the pharmacy which program is processing your prescription and where the prescriber should direct any authorization request. Read the bulletin’s scope and procedures.
Use the plan name from the current membership card, not an old pharmacy receipt. Ask the proposed dispenser to verify participation in that plan. A provider’s ability to ship to Pennsylvania and its ability to bill your Medical Assistance benefit are different facts that must both be established.
Prepare a product specific request
Ask the prescriber and pharmacist to agree on the complete product name before the request is submitted. The name should distinguish the sensor generation and any separate reader or transmitter. If you use an insulin delivery system, have the treating team verify the exact compatible component.
- Identify whether this is an initial request, a refill or a change of system.
- Ask who will submit the authorization and who will answer follow up questions.
- Confirm whether an existing authorization needs to be changed.
- Keep the date of the last supply and any approval reference available for a private provider call.
- Ask how you will receive the decision.
This avoids repeatedly submitting a request that names an incomplete or mismatched setup.
What happens when more information is requested
Pennsylvania’s general pharmacy prior authorization guidance is the official starting point for submission procedures. Ask the prescribing office to use the current process and to keep a copy of the response. If documentation is requested, ask what exact question the reviewer needs answered and which office holds the relevant record.
A request for information is not the same as a final denial. Conversely, an informal assurance that a product is usually covered is not an authorization. Keep each stage clear in your notes. If a formal denial arrives, follow its instructions and deadlines with help from your prescriber or plan rather than trying a different supplier without addressing the stated reason.
Plan the next supply and any model change
Record the authorization end date separately from the next refill date. Ask the dispenser how much lead time it needs for a prescription renewal and how it will tell you if the prescribed product changes. If a device is discontinued or the clinical team recommends another system, ask whether a new authorization is needed before sending a replacement prescription.
Do not arrange overlapping supplies from two dispensers. Contact the current provider about pending claims before transferring. If a sensor fails early, use the manufacturer’s official support process and tell the dispensing pharmacy which event is being reviewed. Keep the replacement case and ordinary refill schedule distinct.
Questions for a benefits review
Ask whether the chosen pharmacy is an appropriate enrolled dispenser, whether the exact component is preferred, whether another insurance benefit must be used first, and which review route applies if the request is denied. For a child, ask the clinical team whether pediatric review provisions affect the request. For someone with an existing pump, prioritize verification of compatibility over a generic brand preference.
A MedraxDME callback can begin a discussion about the next steps, but it is not a Pennsylvania authorization submission. Please keep medical records and insurance identifiers out of the public contact form.
