DME AND INSURANCE INFORMATION

CGM coverage through Ohio Medicaid

Ohio has a dedicated CGM equipment rule and a separate pharmacy benefit system. Start by identifying the route your plan uses for the prescribed device, then confirm documentation and refill arrangements.

General information, not medical advice or an insurance coverage decision. MedraxDME operates as an out of network supplier. Coverage, supplier enrollment requirements and patient responsibility depend on the plan and an individual review.

What Ohio’s equipment rule covers

Ohio Administrative Code 5160-10-36 addresses CGM purchases and replaceable components. It identifies a certificate of medical necessity, ODM 10277, and documentation of the condition, intended monitoring period and reason for the device. It treats integrated insulin pumps and disposable tubeless insulin devices under other rules.

That distinction matters when several devices are used together. Ask the prescribing team whether the request concerns the CGM, the insulin pump, or both. Do not assume approval of one component establishes benefits for the entire system. The office should identify the correct pathway for each requested item.

Find the right pharmacy or equipment contact

Ohio’s Single Pharmacy Benefit Manager portal publishes pharmacy resources and current processing updates. Use it with your pharmacist to locate the appropriate pharmacy requirements. For a medical equipment claim, ask your plan which authorization or documentation process and supplier network applies.

Have the current membership card ready for a private conversation with the plan. Ask which department can confirm the route for the exact CGM, whether the dispenser is eligible to bill it, and what the prescriber should send. A broad answer that “Ohio covers CGMs” does not resolve the device, provider and claim details for an individual request.

Document the request without making treatment assumptions

Ask your clinician to explain the goal of the prescribed monitoring and the equipment needed to support it. For administrative purposes, identify whether this is an initial system, a replacement component or a routine supply. Ask the office to use the exact product name and to keep its documentation consistent with the prescription.

  • Which office will complete any medical necessity form?
  • Where should that office send the prescription?
  • Who will respond if additional information is requested?
  • How will you learn the outcome?

The clinician should decide medical appropriateness. A benefits guide is not a reason to change insulin, stop monitoring or replace professional instructions.

Check the refill confirmation process

The Ohio DME rule requires contact to verify the person’s current need before additional replacement components are dispensed. It also distinguishes monthly units from the physical components inside a supply shipment. See the rule’s constraints and limitations.

Ask the dispenser what information it needs before the next refill and how it will contact you. Keep an accurate count of remaining unopened supplies and tell the provider if a prior shipment is still on the way. Do not agree to duplicate supplies from a second provider while the first request is unresolved. If a phone number changes, update the provider through its normal contact process.

Handle a device change or early failure separately

If the clinical team recommends a new model, ask whether a different prescription or authorization is needed before the next supply is processed. Confirm whether existing receivers or apps remain compatible, especially when a pump is involved. The prescribing team and manufacturer guidance should resolve that question.

For an early sensor failure, use the manufacturer’s support route and keep its case number. Tell the dispensing provider which component failed and whether the manufacturer is arranging a replacement. A warranty case should not be confused with a routine monthly supply request. If you need an alternative monitoring plan while waiting, contact your treating clinician.

What to do when a request stalls

Ask whether the obstacle is a missing prescription, incomplete medical necessity information, an authorization decision, an early refill, a provider participation problem or coordination with other insurance. Request the actual response so the responsible office can address the correct issue.

If you receive a formal denial, read the review instructions and deadlines with your prescriber or plan representative. For children, ask whether pediatric review provisions are relevant. For someone with Medicare and Medicaid, confirm the payer order. Keep these decisions in the appropriate clinical and plan channels rather than entering sensitive details into a public inquiry form.

MedraxDME is an out of network supplier. This educational page does not establish Medicaid enrollment, a contract with your plan, or your eligibility to receive equipment from us. Confirm the dispensing provider with your program before transferring a prescription. Our online form requests contact details and optional equipment selections. Do not enter insurance numbers, medical records or glucose readings.

Questions and answers

Are an insulin pump and a CGM reviewed under the same Ohio equipment rule?

Not always. Ohio’s CGM rule points integrated pumps and disposable tubeless insulin devices to separate rules. Ask the plan to identify the applicable pathway.

Can a CGM request include payment for a mobile phone?

The Ohio DME rule excludes payment for the phone and associated phone services, even when the CGM can use one as a receiver.

Does MedraxDME’s catalog establish Ohio Medicaid participation?

No. The program and proposed dispenser must verify an appropriate enrollment and billing arrangement.
