General information, not medical advice or an insurance coverage decision. MedraxDME operates as an out of network supplier. Coverage, supplier enrollment requirements and patient responsibility depend on the plan and an individual review.
Understand the authorization distinction
Michigan’s MMP 23-31 CGM bulletin removes prior authorization for type 1 diabetes and diabetes during pregnancy, childbirth and the postpartum period when coverage and documentation standards are satisfied. Other clinical situations, including type 2 diabetes, require review under the policy. This is not a blanket approval of every device or every request.
Ask your plan or dispensing provider which part of the policy applies to your request and which current documents it needs. The treating professional should determine the clinical basis. Do not describe a diagnosis or treatment differently simply to fit an administrative pathway.
Check the current manual and your health plan
The MDHHS Medicaid Provider Manual page is the official starting point for current policy. Use the medical supplier chapter together with applicable updates. If you are enrolled in a Medicaid health plan, ask that plan to confirm the current billing, participating provider and submission process.
A policy bulletin explains a statewide change, while the plan and dispensing office still need to process your particular request. Ask the office which manual edition and update it used if its answer conflicts with another source. Keep the actual document link rather than relying on a search snippet or a copied checklist from another state.
Give the prescriber enough time to assemble records
The CGM bulletin addresses recent documentation, insulin or pump information and the treatment plan. It describes six month initial orders, another six month order when use continues, and longer replacement supply orders after the first year. Review the documentation section with the prescribing office.
Ask which office owns the next step and when it expects to send the prescription. If records are held by a different clinician, ask the offices to arrange an appropriate transfer. Keep your own administrative note of who is handling the request without uploading medical details to a general website form.
Tell the dispenser about other insurance
MDHHS lists a separate bulletin, MMP 24-47, for pharmacy secondary payer coverage of selected CGM and insulin delivery products. This is a coordination rule, not evidence that all Michigan Medicaid CGMs became unrestricted pharmacy benefits. The official 2024 bulletin directory identifies that update.
If you also have Medicare or private insurance, ask the dispensing provider which payer must process the first claim and whether the secondary route is appropriate for the exact device. Do not cancel existing coverage or change a dispenser merely to obtain a different billing route. The offices should explain the sequence before sending supplies.
Keep the equipment identity clear
Read the full sensor name from the prescription or package, including the model generation and any “Plus” designation. Ask whether you need a receiver, a separate transmitter or only the recurring sensor supply. If an insulin pump is used, have the prescribing team verify compatibility before the pharmacy changes the sensor.
- Identify whether this is a first system, refill, replacement or model change.
- Ask which components are included in the request.
- Confirm whether an existing order needs revision.
- Ask who provides training and where device questions should go.
A familiar brand name does not necessarily identify the exact approved configuration.
Prevent avoidable refill and transfer problems
Ask the current dispenser whether a shipment or claim is pending before changing providers. Ask the new provider to confirm participation in your program and the next permissible processing date. Record the prescription and authorization end dates separately from the refill schedule.
If a sensor fails, contact the manufacturer’s official support and tell the dispenser whether a replacement is being arranged. If a request is denied, keep the notice and use its correction or appeal instructions with your clinical team. For a child receiving Children’s Special Health Care Services, ask the clinician to check that program’s additional requirements. If monitoring supplies may run out, seek guidance from your treating professional.
MedraxDME is an out of network supplier. This educational page does not establish Medicaid enrollment, a contract with your plan, or your eligibility to receive equipment from us. Confirm the dispensing provider with your program before transferring a prescription. Our online form requests contact details and optional equipment selections. Do not enter insurance numbers, medical records or glucose readings.
Questions and answers
Does “no prior authorization” mean no documentation?
No. Michigan’s exception still requires the applicable coverage standards and documentation. The dispensing provider must verify the request.
Does the secondary payer pharmacy policy apply to every CGM request?
No. It concerns selected products and coordination with primary coverage. Ask the provider to confirm the appropriate route.
Is MedraxDME guaranteed to be an eligible Michigan Medicaid dispenser?
No. Enrollment, plan participation and the particular billing arrangement require separate verification.
