General information, not medical advice or an insurance coverage decision. MedraxDME operates as an out of network supplier. Coverage, supplier enrollment requirements and patient responsibility depend on the plan and an individual review.
Avoid older restrictive criteria
The HFS criteria effective December 1, 2024 describe pathways for type 1 diabetes, type 2 diabetes requiring insulin, and gestational diabetes, with training attestation. They also provide for individual medical necessity review outside those listed pathways. Older online forms may contain requirements that no longer match this document.
Ask the office handling the request to identify the version it is using. A search result’s publication or crawl date does not necessarily mean the underlying clinical criteria are current. Keep a link to the actual HFS document and ask the prescriber to resolve any conflict with the payer before resubmitting the same information.
Separate the authorization period from the dispensing schedule
HFS’s November 2024 provider notice confirms that a valid CGM authorization is approved for at least 12 months and applies to fee for service and managed care. The criteria include specific pregnancy related duration provisions. An authorization period is not a direction to dispense a year of sensors at once.
Ask the pharmacy to explain the actual refill interval and how it will request confirmation before the next supply. Keep the end of the authorization on your calendar, alongside the follow up dates recommended by your clinician. Those dates serve different purposes and should not be treated as interchangeable.
Choose the correct HealthChoice or fee for service contact
Look at the current membership card and ask which program handles the CGM request. Then confirm the appropriate authorization department and an eligible dispensing provider. If the plan says to contact a pharmacy, ask whether a particular network or preferred product requirement applies.
A useful call record includes the department contacted, the date, a reference number and the agreed next step. If a representative redirects you, ask what information the next office needs. This helps prevent repeated calls in which each office assumes another has already submitted the prescription or clinical records.
Make the prescription and device choice match
Write down the complete name of the prescribed sensor, not only its manufacturer. Ask whether a receiver or separate transmitter is part of the request. If you already use a CGM, take the existing package to the clinical appointment so the team can distinguish a routine refill from a change of system.
- Confirm who will provide any device training.
- Ask the prescribing office to check compatibility if a pump is involved.
- Ask the pharmacy whether the exact item is on the current list.
- Identify any prior authorization that must be updated rather than duplicated.
A request should reflect the clinician’s actual assessment. Do not add symptoms, treatment failures or insulin use solely to fit a coverage rule.
When the answer appears to use an old rule
The Illinois Public Aid Code provision for CGM rules is another official reference for the state’s changes. If a rejection cites an older intensive insulin or hospital visit requirement, ask the prescriber or plan to explain which current policy applies and whether the request was evaluated under the correct program.
Keep the actual denial notice and response deadlines. Ask whether the issue can be resolved by correcting the request, providing clinical information or using the formal review process. A general guide cannot overturn a decision, but it can help you ask the right question and locate the official rule the reviewer used.
Before you change dispensers
Ask the existing provider whether it has an open claim or pending shipment. Ask the proposed provider to verify participation in your plan and the next eligible processing date. Transfer arrangements should be coordinated between the appropriate offices rather than triggered by filling out several product inquiry forms.
For a child or a pregnancy related request, have the treating team check the applicable criteria and product labeling. For Medicare alongside Medicaid, ask which payer will handle the first claim. If supplies are running low, contact your clinician about a safe plan while administrative questions are resolved. This website does not provide an emergency monitoring or treatment service.
MedraxDME is an out of network supplier. This educational page does not establish Medicaid enrollment, a contract with your plan, or your eligibility to receive equipment from us. Confirm the dispensing provider with your program before transferring a prescription. Our online form requests contact details and optional equipment selections. Do not enter insurance numbers, medical records or glucose readings.
Questions and answers
Should I use a 2021 Illinois CGM criteria sheet found online?
Ask the provider to use current HFS criteria and plan procedures. Older search results can contain superseded requirements.
Does a 12 month authorization mean one shipment for the entire year?
No. Authorization duration and dispensing quantities are different. Ask the pharmacy to explain the refill schedule for your request.
Does this guide mean MedraxDME is a HealthChoice contracted provider?
No. Enrollment and participation in the specific plan must be verified separately.
