General information, not medical advice or an insurance coverage decision. MedraxDME operates as an out of network supplier. Coverage, supplier enrollment requirements and patient responsibility depend on the plan and an individual review.
Use the current DCH source, not an old brand list
The Georgia Department of Community Health maintains a prior authorization criteria directory for its fee for service and PeachCare outpatient pharmacy program. The directory includes a CGM entry posted February 10, 2026. It also links the request process and related forms. The exact current document should be reviewed by the prescribing office and dispensing pharmacy.
Public search results can show older CGM summaries and different versions of product lists. This guide does not treat an older brand table as evidence that every current device is covered or excluded. Ask the pharmacist to check the exact product number and the current program rules before changing a prescription.
Distinguish Georgia Families from fee for service
Georgia’s Care Management Organizations page identifies the managed care route and plan contacts. If your card names a care management organization, ask that organization how the CGM request should be handled. Do not assume a fee for service pharmacy contact manages the same request for your plan.
Write the program and plan name at the top of your call notes. Ask the representative which benefit processes the device, who receives the prior authorization and which dispensing providers participate. If you are redirected, ask for the receiving department and the reason. This reduces the risk of the prescription repeatedly moving between an equipment supplier and a pharmacy.
Make the product check specific
DCH’s preferred drug list page provides current lists and archive links. Ask the dispensing pharmacy to use the appropriate current product and diabetic supply resources, not to infer CGM coverage from an insulin approval or a general diabetes diagnosis alone.
- Read the full name of the sensor from the prescription or current package.
- Ask whether a receiver or separate transmitter is needed.
- Identify whether the request is new therapy, a refill or a switch.
- If an insulin pump is used, ask the treating team to confirm the compatible sensor.
- Have the pharmacy confirm any product number or quantity issue before requesting a clinical change.
Organize the prescriber and pharmacy handoff
Ask the prescribing office who will submit the request and who will answer questions from the reviewer. Ask the pharmacy what it has already received. If the two offices disagree, a clear list of missing items is more useful than another generic request for “all records.”
Keep your own administrative checklist with the date sent, the office responsible, a reference number if provided and the next follow up date. Clinical records, glucose readings, diagnoses and insurance identifiers should be shared through the provider’s appropriate secure process. The MedraxDME public form is not a substitute for a Georgia Medicaid authorization submission.
Treat approval, dispensing and delivery as separate steps
Even after an approval is issued, ask the dispensing provider to confirm the exact item, the permitted processing date and whether it can obtain the product. An authorization does not itself confirm stock or establish a delivery date. Do not stop an existing supply arrangement until the replacement arrangement has been clarified.
Ask how the provider wants you to request future refills and what to do if your contact details change. If you have other insurance, ask how it will be coordinated. For a child, ask the clinician and plan about the relevant pediatric review rules and device labeling. Avoid treating another family’s approval as proof that the same pathway applies to you.
If the answer is unclear or the request is denied
Ask for the specific reason and the current policy reference. A wrong product number, an ineligible dispenser, a missing authorization and a clinical denial need different responses. Ask whether the pharmacy can correct a processing issue or whether the prescribing office must submit additional information.
For a formal denial, keep the full notice and follow its appeal instructions and deadlines. If a sensor has failed, contact manufacturer support separately and keep that case distinct from a routine benefits request. Contact your treating professional if you are at risk of a gap in your prescribed monitoring.
MedraxDME is an out of network supplier. This educational page does not establish Medicaid enrollment, a contract with your plan, or your eligibility to receive equipment from us. Confirm the dispensing provider with your program before transferring a prescription. Our online form requests contact details and optional equipment selections. Do not enter insurance numbers, medical records or glucose readings.
Questions and answers
Is Georgia’s fee for service CGM document enough for a managed care request?
No. Ask your care management organization to confirm the route, current criteria and participating dispenser for the request.
Can I assume an older preferred brand list is still current?
No. The pharmacy should verify the current product number and program materials. Older search results can remain visible after an update.
Does this guide say MedraxDME is enrolled in Georgia Medicaid?
No. Enrollment and an appropriate plan billing arrangement have to be confirmed separately before supply can be arranged.
