DME AND INSURANCE INFORMATION

CGM coverage through California Medi-Cal

For a California CGM request, first identify whether the prescribed system belongs in Medi-Cal Rx or a managed care medical benefit. That distinction determines who can dispense it and where the authorization goes.

General information, not medical advice or an insurance coverage decision. MedraxDME operates as an out of network supplier. Coverage, supplier enrollment requirements and patient responsibility depend on the plan and an individual review.

Start with the Medi-Cal Rx route

The October 2026 Medi-Cal Rx provider manual places contracted CGM systems in its pharmacy medical supply benefit, with prior authorization. A noncontracted system may instead be considered through a managed care plan’s medical benefit. This is a channel distinction, not a promise that either route will approve a particular product. See section 13.3 of the Medi-Cal Rx manual.

Before calling several suppliers, ask your pharmacy to identify the exact system and confirm the correct route. Write down the complete product name, including “Plus” or a particular wear duration when present. A familiar brand name alone may describe several different products.

Match the exact component, not only the brand

The official covered products directory links the contracted CGM list. Use that list with your pharmacist to check the sensor, receiver and any separate transmitter. Ask which item is being prescribed now: a first system, a refill, a replacement receiver, or a change of model.

  • Read the product name from the existing package, rather than relying on the app name.
  • Ask the pharmacist to confirm the billing product number.
  • If a pump is involved, ask the prescribing team to check compatibility before any substitution.
  • Confirm whether an app, reader or other component is needed for your prescribed setup.

A listed product and a clinically appropriate product are separate questions. Your treating professional should resolve the clinical choice.

Prepare for the authorization conversation

The current manual describes clinical pathways involving insulin use, documented problematic hypoglycemia or pregnancy related diabetes. Its renewal process also requires clinical follow up. The precise circumstances belong in the prescriber’s records, not in a retailer’s public contact form. Read the CGM authorization section with the prescribing office.

For an efficient call, ask who is responsible for submitting the request, whether the office has received the pharmacy’s request, and how you will learn the decision. You do not need to interpret clinical thresholds yourself. If the office needs information from another clinician, ask it to arrange an appropriate record transfer.

Keep approval dates separate from refill dates

A useful personal tracking sheet has four entries: the approval reference, its end date, the date of the last supply, and the next date the dispensing pharmacy tells you a refill can be processed. A long authorization does not mean all future supplies arrive automatically. Ask the pharmacy how it wants you to request the next supply and what to do if you will be away from home.

If a component fails before the expected end of use, contact the manufacturer through its official support route and tell your pharmacy what happened. A manufacturer replacement and a routine insurance refill are different requests. Avoid arranging duplicate shipments while both are being reviewed.

Resolve a pharmacy or medical benefit mismatch

DHCS explains the separation between Medi-Cal Rx and other medical benefits in its Medi-Cal Rx questions and answers. If one office sends you to another, ask for the reason in plain language: the item is outside the contracted list, the claim needs authorization, the pharmacy is not the correct dispenser, or another payer must be billed first.

Keep the actual rejection or notice. A message that a pharmacy cannot process a claim is not necessarily a final decision that no CGM is covered. Ask the prescriber which policy and component the request used before starting again with a different device. If a formal denial arrives, follow the review instructions and deadlines printed on that notice.

What to confirm before switching suppliers

Ask your current pharmacy whether a pending refill or authorization will be affected by a transfer. Ask the proposed dispenser to confirm its Medi-Cal billing eligibility for your exact request. Do not cancel an existing arrangement solely because a website has a product photograph.

For a child, ask the prescribing team to check the applicable pediatric coverage rules and the device’s authorized age range. For someone who also has Medicare, ask which benefit is primary and whether a separate coordination step applies. These details can change the correct pathway even when the sensor brand stays the same.

MedraxDME is an out of network supplier. This educational page does not establish Medicaid enrollment, a contract with your plan, or your eligibility to receive equipment from us. Confirm the dispensing provider with your program before transferring a prescription. Our online form requests contact details and optional equipment selections. Do not enter insurance numbers, medical records or glucose readings.
